INSURANCE PROCEDURES


Injuries

1.  All injures to players should be noted on the referee report.  This will help confirm to the insurance company that the injury occurred at one of our league activities.  Having said that, injuries don't always manifest themselves until later in the day; or even the next day.  We can still file with our league insurance, but if possible it is best to have the injury noted on the report.  The insurance form requires a witness, which can be someone who was at the game, in case of a problem though, it is helpful to have the injury noted in the referee report.

2.  The insurance injury form should be filled out within a few weeks of the injury - even though all bills probably have not been received.  This will serve as a notification to the insurance company; and they will get a file open.  If you delay, you run the risk of them not covering your medical costs. 

3.  Please send me a copy of the form that you send to the insurance company.  I don't need a copy of all the bills for associated expenses, but need a copy of the form itself.

Information need for USL insurance  certificates

A)   Field Info:
   We need the following information:

     1)  Field owner  (Ex:  Washington Township)
     2)  Owner's address:  (Ex.  10 Rock Rd., Long Valley, NJ 07853)
     3)  If possible, Recreation Director's name (Ex. Attn: Tom ?)

B)  Team Manager:  We need the team manager's name and address.

C) Team Name:  We need the exact team name since this gets put on the insurance certificate.

D)  Teams using Bergen County Fields:  Bergen County requires the certificate to include all of the names of a teams' competitors as well.  We will obtain these once Divisions are finalized, and will have all your competitors included on your certificate.

E)  Number of Certificates and Certificate Fees:  We haven't reached a limit on the number of certificates that a team can obtain - yet.  However, we have had occasions where our carrier has attempted to charge a fee for additional certificates when we have requested certificates later in the season.  So far they have waived the fees; but, please make a note of this.
